Subcontractor Prequalification

Thank you for your interest in working with Cardinal Construction. Please complete the following form to the fullest extent possible. Upon receipt of submission, a representative from Cardinal will be in touch to review next steps.

List your Company's # of Injuries from your OSHA Logs as follows:

Please note all projects under contract, including those not yet started. Include start and completion dates, and contract values for each. Attach an additional sheet if necessary.

Please provide at least three (3) examples of relevant project experience. Information should include project name and location, a description of scope, initial and final project schedule, initial and final contract value. Please also include the name of the General Contractor or Construction Manager, with contact information (name, title, phone and email) for each project.

Please provide resumes or brief biographies for key personnel likely to be involved in the procurement and construction aspects of the project.

Submission of financial states is not required at this time. However, we reserve the right to inquire upon receipt of this information. All information will be handled confidentially.
